Ready to join? Please register to enroll in our CE Co-sponsorship Program. After registering, you will receive a confirmation email with links to the Pre-Course and Post-Course Application Forms, along with a list of required materials for each.

Pre-Course Application Form & Required Materials

Upon enrolling in the CE Co-Sponsorship program, please submit the CE Co-Sponsorship Pre-Course Application Form at least 7 days prior to the start of the course.

Attendance Sheet

  1. Attendance sheet to be signed by all participants on the last day of the course.
  2. Daily or Weekly Sign-in/out sheet for participants requesting CE credit. (Daily sheet for 1–6 day programs; Weekly sheet for weekly programs.) License information is required.

Official CE Statement

The official CE statement must be included in all promotional materials (website, flyer, email announcement).

Promotional Materials

Promotional materials should include:

  • Course description and objectives
  • Dates and location
  • Course fees
  • Teacher(s)' bio(s)

Please review the MBSR sample evaluation form for reference. Evaluations must include the following required questions:

Post-Course Application Form & Required Materials

Submit the CE Co-Sponsorship Post-Course Application Form within 2 weeks after course completion.

⚠ After submitting the Post-Course Application Form and required materials, please email mindfulness@health.ucsd.edu. CE fees will be charged to the credit card on file.

CE Certificate Template

Provide a CE certificate to each participant who requested CE credit upon completion of the course.

Scanned Attendance Sheet & Sign-in Sheet

Submit the Weekly Sign-in sheet signed by all participants. For online courses, the instructor may mark attendance for all participants. License information is required for participants who obtained CE credit.

Evaluations

Submit a summary of evaluations. Evaluations must include the following required questions:
